Notes on Issuance
  • Verification of the patient's identity is required when issuing copies of medical records and certificates.
  • When a family member or a proxy, rather than the patient in person, applies, documents such as an ID, a consent form, a power of attorney, and family relationship verification documents may be required in accordance with the relevant laws.
  • The issuance of copies of medical records and certificates is officially accepted after completing the application form, identity verification, review of required documents, confirmation of the scope of issuance, fee guidance, and payment confirmation.
  • Requests made by phone, text message, KakaoTalk, personal contact, or verbally are handled as inquiries or application guidance, and the processing time is calculated from the date the official application is completed.
  • Documents that require preparation or confirmation by the attending physician, such as medical certificates, doctor's opinion letters, and surgery confirmation certificates, may take about 3-5 business days depending on the attending physician's treatment schedule, surgery schedule, non-clinic days, weekends, public holidays, and the scope of record review.
  • If the scope of the issuance request is broad or you request all past medical records, additional time may be required for review and printing.
  • Documents that are not separately prepared and kept at our clinic, materials that are not subject to standard copy of medical records or certificate issuance under the law, internal work memos, communications between staff, and materials containing others' personal information may be restricted from or unavailable for issuance.
  • If you request a document under a separate title, it will be issued within the scope of verifiable records actually kept at our clinic, such as medical treatment records, surgical records, nursing records, test results, prescriptions, and consent forms.
  • In the case of anesthesia records, the information is provided within the scope recorded in the medical treatment records, surgical records, and nursing records, instead of as a separate document.
  • Initial visit charts and follow-up visit charts are included in the medical treatment records.
  • To protect personal information, we do not provide viewing by directly showing the electronic medical record screen; when viewing is requested, after identity verification and confirmation of the viewing scope, we prepare a copy or printout for viewing purposes.
